I've only noticed this problem with Collection category achievements. So far, I've been awarded the following achievements without actually meeting the requirements:
- Ten Tabards (https://www.wowhead.com/achievement=1020/ten-tabards)
- Twenty-Five Tabards (https://www.wowhead.com/achievement=1021/twenty-five-tabards)
- Fashionista: Tabard (https://www.wowhead.com/achievement=10690/fashionista-tabard)
- Fashionista: Shirt (https://www.wowhead.com/achievement=10691/fashionista-shirt)
- Fashionista: Weapon & Off-Hand (https://www.wowhead.com/achievement=10689/fashionista-weapon-off-hand)

I suspect the issue is caused by something in the quest system because I'm almost 100% certain all of them popped when I handed in completed quests. Unfortunately, I wasn't expecting a bug like this to occur, so I wasn't paying enough attention to the quests I was handing in to remember what their names were. I think the Fashionista: Weapon & Off-Hand popped when handing in some quest in Un'Goro Crater, and Fashionista: Tabard and Fashionista: Shirt popped during questing in Teldrassil. For the other 2 achievements I unfortunately don't remember any additional details.
